Abdominal Aortic Aneurysm Case

Patient

70 yr old male with abdominal pain. Medical history of HTN. Onset 1 hour earlier. No CP, vomiting or diarrhea. Just started chemotherapy for lymphoma. Physical exam BP 96/60, HR 90, resp 16, Sat 100%. Slightly pale but not diaphoretic. Lungs clear, Abd diffusely tender, A&Ox3. No additional PE provided.

ED Course

IVF, fentanyl, O2 and transport. En route pt becomes confused. Pt arrives lethargic with BP 70/40, HR 105. PE abdomen is distended and tender. Palpable pulsatile mass and unequal femoral pulses. Primary concern? Bedside ultrasound shows Abdominal aortic aneurysm and free fluid. Labs sent, type and cross for blood, vascular surgeon called. CT confirms ruptured AAA.
