Request Your Medical Record

Patients may request a copy of their Sky Ridge Medical Center medical record from the Release of Information area in our Health Information Management office. In order to request your medical record, download and complete a medical records release form (below) and submit the completed form via mail, fax or in person. Records are made available within 10 days of your request.

If you cannot download the form, please submit a written request to our address or send a fax that includes the patient's name, date of birth, social security number, requested information, and dates of service. It must be signed by the patient and dated.
